CA Lottery: Do I have to put SS# for prize claim form.Prev TopicNext Topic

  Hello, I know in CA, undocumented people can still claim prizes. The CA claim form has a check box that says " I don't have an SS#". They simple are subject to 28% or 30% withholding Vs 25% for SS# holders.

   

  My question is I do have an SS#, but I don't want to write on the claim form with all this ID theft going on. Even at the Drs office I never put my SS# on their forms.

   

  My address is the same for the last 23 years, this should suffice right? I'm sure they can "find" me and lookup the SS# themselves.

   

  What do you folks think?  Thanks.

  Your SS# on the form is for tax purposes. Doctors use it for patient identification. If you turn it in without it, eventually they will "find" you as you put it, when you file your taxes next year. But why do you want to risk having your winnings held up, an IRS audit, or them questioning why you left it off? In other words, why open up a can of worms if you don't have to. But if you should be lucky enough to win the jackpot, then you can ask your tax accountant and get professional advice about that. 

  Failure to provide your original signed winning ticket, social security number, date of birth, name and complete address (including apartment or space number), city, state, zip code and phone number may delay or prevent the Lottery from processing your prize claim or result in additional federal taxes withheld from your prize. Lottery prizes are not subject to California state income tax.

  
  I declare, under penalty of perjury and the laws of the State of California including but not limited to California Penal Code sections 118 and 72, that I am the rightful owner of the winning ticket on this form, that I am 18 years of age or older and that all information provided is true and correct. I understand that any person who, with intent to defraud, falsely makes, alters, forges or counterfeits a Lottery ticket is in violation of state law and could be liable for criminal penalties.

  Sounds like your intent is Tax evasion which often entails taxpayers deliberately misrepresenting the true state of their affairs to the tax authorities to reduce their tax liability and includes dishonest tax reporting, such as declaring less income, profits or gains than the amounts actually earned, or overstating deductions!

  Whoa... California.... tsk tsk tsk , your volunteer info sections need severe updating...no section for indigenous people ..only male or female for the gender ?...not good
